The objective of this study was to develop a technique for selective isolation of metastatic cancer cells from the blood of patients with metastatic melanoma using immunoadsorption on anti-melanoma antibody-Sepharose. The technique was evaluated using an anti-melanoma monoclonal antibody conjugated to Sepharose, and metastatic cells were isolated from peripheral blood obtained from melanoma patients. Cells from positive controls (mouse colonic carcinoma, NSG mouse melanoma) were isolated in 80% to 90% yield and purity. Metastatic cells from patients with metastatic melanoma were not recovered in similar numbers. The yield of positive cells was strongly dependent on the antigen used for the immunoadsorption. Decreased yield and purity were obtained when whole blood was used instead of purified mononuclear cells. The yield and purity of positive cells was further decreased when tumor cell growth was inhibited by incubation with vincristine. The technique did not isolate the melanoma cells if cell growth was inhibited by treatment with anti-IL-2 antibody. Our findings suggest that Sepharose conjugated to anti-melanoma antibody does not react with the melanoma cells. However, some reactivity of the Sepharose conjugate with normal blood cells exists. This phenomenon has been shown in in vitro experiments and may be responsible for some of the low yields obtained when the technique is used on whole blood samples.
